What is the Chronic Condition Self-Management Program?
It is a six week course that teaches participants to better manage emotion and physical challenges.  This is done through offering information on exercise, healthy eating, communication and goal setting.  Self Management is the positive effort of an individual to seek access, oversee and participate in his or her health care to optimize health, prevent complications, control symptoms, and minimize the intrusion of symptoms in a healthy productive life.

What happens if you don’t find me a job?
We don’t find people work but rather assist in people in finding work for themselves. We understand that employment is difficult to secure and therefore offer weekly, focused vocational counseling and resources to achieve success.

Will I have my own room?
Yes, all residents have their own room and a key to the front door and to your bedroom.  Common areas including bathrooms and kitchen are shared.  The kitchen is also shared with staff and Workshop Fellows during business hours.

What happens if you don’t find me housing after nine months?
We don’t find housing for our clients; instead we provide resources to aid in the process. If no housing is found after the resident demonstrates they have performed a thorough search, we offer a three-month extension. If, after the extension, the resident is still unable to secure independent housing, we can refer to other housing within the Conard system.

Why should I enter this program?
We are specifically looking for people who would like to exit the supportive housing system and live in a completely independent setting. Many people living in supportive housing and/or on limited incomes aren’t able to move forward with personal aspirations because of housing and financial constraints. It can be incredibly difficult to exit public assistance and/or supportive housing. JSC offers residents the opportunity to challenge themselves to move forward with their goals and take control back in their lives.  Because everyone deserves a chance to live independently, get married, own homes and have families if this is what they choose.

Do I have to leave my clinic/therapist/psychiatrist if I enter the program?
You don’t need to leave your current treatment team. We encourage residents to stay connected with their providers and work in tandem with JSC staff to provide the best services.

Do I need to attend workshops?
Workshops are offered Monday through Thursday at the Jackson Street house and open to all Conard House clients including hotel and Co-Op residents. Residents are encouraged, but not required to attend the workshops.  There is also a weekly evening group, for residents only, to process difficulties, problems, progress and other issues related to benefiting from the program. There is an expectation that residents will make use of this group to assist them in achieving their goals.

What will my day structure look like?
We ask that residents meet individually with JSC staff and attend a weekly meeting with the residents. We expect residents to be working, attending school, or actively seeking employment, while they are at JSC.

Do I have additional bills to pay while in the program?
PG&E, Comcast, garbage and water bills are all included in the rent.  We also offer two computer workstations with internet and printer for all residents to use.  In addition there is laundry on site at no cost to the residents.  There is a comfortable living room area exclusively for residents use with a television and cable for relaxation and community.
