Jackson Street Community for Supported Self-Management
Opportunities to move beyond supportive housing for people who feel they are ready.
A new nine-month resident fellowship program designed to prepare and empower people transitioning from supportive housing towards recovery oriented goals. Residents will live communally with 8-10 other residents strengthening independent living skills. Residents will work with staff to accomplish individual vocational and career goals. Residents will transition after nine months to independent housing outside of the mental health system.
Residential Fellowship Program Requirements:
- Age 18 or older and currently receiving services through CBHS.
- Must be one of the following: a) currently employed, b) have a solid work history and a desire to work, c) in school and work-ready within nine-months or d) have a feasible plan for moving out of the mental health system.
- Completion of the Chronic Disease Self Management Course or willingness to attend.
- Demonstrated basic self-management skills and motivated to self-manage health care, behavioral health, employment/vocational and independent housing goals.
- Willingness to commit to the nine-month residential fellowship program, to participate fully in a co-ed community of 8-10 residents and to transition to independent housing upon completion.
- Ability to pay rent of $500/month plus a monthly security deposit of $83.33/month for 6 months.
- Conard House will match up to $2,000 through rent reimbursement. This money can be used for move out costs.
- Secure employment within three months of entering the program and work closely with the Vocational Counselor to achieve this goal.
